I sent a check to the Social Security Administration to give me some information about past employers because I'm trying to prepare to look for a new job. Specifically, I was looking for the exact dates (mm/dd/yy) of my employment periods. Some online applications require mm/dd/yy of periods of employment and would not let you continue until you put in something. I could make up the days, but what if they find out that I had lied. That wouldn't look good. Furthermore, some applications, such as those for government jobs, which I am considering, require mm/dd/yy.

Examining the report, not only was I shocked to discover that I have had at least 14 jobs in the eight years that I've been active in the workforce (serious work ADD, I know, but to defend myself, most of those jobs I had while in school), but also disappointed to discover that Social Security only provided me with the years of employment for each employer.

Is there some other kind of service that allows me to obtain the dates of employment period elsewhere? Or should I just call each company and ask them for the information?

Honestly, I don't think you should worry about it too much. I always put a range on applications, because it's hard for me to often remember specific dates (ie. 4/04-4/05). The HR people are going to call and get the specific starting/ending dates anyways. If it gives you peace of mind though, do try to get in touch with your three most recent employers. That's usually all most employers are concerned about anyways.

I seriously doubt a future employer is going to think you intentionally lied about dates of employment if you are somewhat close to the correct dates.

You can always contact the former employers and ask, "so, when did I work for you?"
